Mayor Bowser to Host Form-a-Palooza

(WASHINGTON, DC) – On Saturday, July 22, at 11:45 am, Mayor Muriel Bowser will host Form-a-Palooza, an engagement forum to improve the efficiency and usability of DC Government forms. The first of its kind, Form-a-Palooza will demonstrate how design and behavioral science can increase accessibility of government forms for DC residents.

Led by The Lab @ DC, the following government forms will be presented at the forum: Department of Motor Vehicles’ Driver’s License and Identification Card Application to obtain or renew a license or identification; Department of Consumer and Regulatory Affairs’ Basic Business License EZ Form to certify a business is authorized to operate legally in the District; Department of Human Services’ TANF Job Search Log to document potential employment; Department of Disability Services’ Disability Services Intake Application to access developmental services; and Department of Energy and Environment’s Lead Disclosure Form to report the possible presence of lead-based paint. The Mayor will also participate in sessions where residents will learn how to generate a new form to increase functionality and key features.

As a part of Back to Basics DC, an effort to highlight the day-to-day work that keeps the District moving forward, the Mayor will continue to recognize the work of DC government employees, showcase innovations in service delivery, and share information about how the community can help the government keep Washington, DC a great place to call home.
